Dr. Iwata is consistently portrayed as a dreadful, lascivious physician whose prominence in the medical field is largely attributed to his family's influence. The most striking aspect of his appearance is an X-shaped scar that spans his entire face, inflicted by his cousin Norikuni Iwata at the tender age of four. This incident has bred a deep-seated hatred towards his cousin ever since. Dr. Iwata's character and appearance are satirical takes on Osamu Tezuka's Black Jack. While several manga chapters center around him or his hospital, he only makes brief cameo appearances in the anime adaptation.
