This chapter clearly stated that:
Ichigo was STRONG enough to bring Kenpachi on the verge of death.
Kenpachi limited himself against Unohana and Ichigo was actually strong enough to increase Kenpachi's power enough for him to own Nnoitora.
I dont think so. Shes constantly taking him through a cycle of near death/recovery. the one fight he had with ichigo wouldnt have had that much of an effect. It seems that its because Unohana is doing the 'killing' that its working. She was the first person he held himself back on. Therefore, if shes the one putting it over him, he'll want to win more and more, thus releasing his shackles. Its more that he was always the strongest out there, he would subconsciously limit himself to the same as his opponents reiatsu, and therefor thinking that they were strong enough to make a good fight. His fight with ichigo makes more sense now. I mean a captain being defeated by a shikai and just winning? The guy who killed the previous captain? We always thought it was ichigo's awesome reaitsu (which was a key part), but if kenpachi was fighting with the strength he used to defeat nnoitra and yammy, ichigo wouldnt have stood a chance in shikai. Hey, maybe aizen knew this about kenpachi, and orchestrated the fight between them (however he did it) just to get ichigo to grow stronger.
So unohana is taking him to the brink of death and healing him over and over. The reason she said one of them would have to die makes more sense now. Kenpachi will eventually be strong enough to deal her a fatal blow in one strike. Then when she's dead, he'll completely unshackle himself and be uberpachi.
