Elizabeth is an Okama from Momoiro Island. He discovered and cared for Sanji when he was unconscious, but upon waking, Sanji hallucinated that Elizabeth was a beautiful young woman and chased after him. This pursuit led Sanji to Elizabeth's frilly home, where Elizabeth tried to convert Sanji into an Okama. Once Sanji recovered from his hallucination, he realized the entire island was filled with Okamas and ran away. Elizabeth and the other Okamas chased after Sanji and eventually caught up with him.
