Orihime Inoue has long orange-brown hair that she wears with the bangs behind her ears and adorned with hairpins. She only removes these hairpins when sleeping, as seen in episode 64. The hairpins are a tribute to her late brother, while her long hair honors a promise made by Tatsuki Arisawa to protect her. Orihime's large breasts often become the subject of jokes within the series. She also possesses an incredibly hard head, which has caused injuries to Uryū, Chad, and Ichigo. Only Yachiru Kusajishi, riding on Kenpachi Zaraki's shoulders, has managed to cause her any harm in a comical moment. In colored manga illustrations, Orihime has yellow-orange/brown eyes, but they appear gray in the anime.
Orihime is friendly, humorous in her unique way, sensitive, and kind-hearted. Despite her apparent naivety and cluelessness, she excels academically. She struggles with technology; one member of the Handicrafts Club remarks that she lacks the intelligence to use a cell phone, explaining why she doesn't own one. Additionally, Tatsuki notes that Orihime has black belt-level fighting skills, though the English dub suggests she is at yellow belt level.
Orihime enjoys cooking and eating unusual food at home, although she also likes some conventional dishes. Her tastes are shared by few, if any, except perhaps Rangiku Matsumoto and possibly Tessai Tsukabishi. Her favorite food is red bean paste, and she enjoys putting butter on sweet potatoes.
Orihime tends to act impulsively, sometimes leading to embarrassing situations. For example, when schoolgirls ask Rukia about her feelings for Ichigo, Orihime expresses disappointment when Rukia denies having any romantic interest, reasoning that it would have been two girls against one boy, ensuring a win for their "team." She also has an overactive imagination, often getting carried away with implausible scenarios, such as envisioning herself as a futuristic and highly destructive robot in response to the question "How do you see yourself in the future?"
Orihime is perceptive, especially regarding Ichigo Kurosaki. She is in love with him, and her feelings become increasingly evident throughout the manga and anime. She has confessed her feelings to both Rangiku Matsumoto and Ichigo, though he was asleep during the latter confession. Orihime lives alone in Karakura Town, supported by relatives who live elsewhere. Previously, she lived with her older brother, Sora, who was fifteen years her senior. They were raised by an alcoholic father and an unfaithful mother, both of whom often argued and physically abused their children. It is unclear if her parents are still alive.
When Sora turned eighteen, he ran away with three-year-old Orihime and raised her independently. For six years, despite being bullied at school for her long hair, Orihime and Sora lived harmoniously. One day, Sora gave Orihime a pair of hairpins, but she initially refused to wear them, finding them childish. On the same day, Sora was injured in a car accident and died at the Kurosaki clinic. Since then, Orihime has worn the hairpins. A year after Sora's death, Orihime met Tatsuki, who defended her from bullies, and they have been best friends ever since.
