Nnoitra is the fifth Espada, marked by the "5" tattoo on his tongue. He stands out as an exceptionally tall and lanky Arrancar, towering over Yasutora Sado by at least a foot, even when leaning over. Nnoitra has long black hair and almost always wears a broad smile that exposes his upper teeth. His attire includes an eyepatch, three bracelets on each wrist, and an oversized hood resembling a spoon on his Arrancar uniform. In the past, his uniform had a normal collar, and his hair was just above shoulder length. The remnants of his Hollow mask and hole are not visible.
Nnoitra comes across as rude and lecherous, highlighted by his snake-like eye and his nickname for Orihime Inoue, "pet-sama". He even inquires Ulquiorra about the extent of Orihime's "training", to which Ulquiorra responds by turning away and calling him disgusting. Flashbacks reveal his belief in male chauvinism, constantly trying to relegate women deemed stronger than men, like Nel Tu, to more "appropriate" inferior ranks. While he initially appears very sadistic, he has no interest in killing weaklings since it doesn't prove his strength. However, he has no qualms fighting strong opponents who are injured and exhausted, a trait both Ichigo and Grimmjow criticize him for.
Despite his disdain for females, Nnoitra shows some concern for his subordinates. When his subordinate Tesla engages Kenpachi Zaraki, Nnoitra yells for Tesla to retreat before he gets killed. However, the prospect of a new battle quickly overshadows this concern.
Nnoitra's zanpakutō, Santa Teresa, takes the form of a massive ax-like weapon with two crescent moon blades fused together. It previously had only one crescent blade. Upon release, Nnoitra gains two additional pairs of arms, each hand wielding an identical scythe-like weapon. In his released state, Nnoitra's wounds heal almost instantly, and he can regenerate severed limbs.
Nnoitra arrives at the battle between Ichigo Kurosaki and Grimmjow Jaegerjaques. As he tortures the exhausted Ichigo, his former companion Nel Tu returns in her adult form and attacks Nnoitra to protect Ichigo. Despite nearly defeating him again, her amnesia returns, leaving her vulnerable. Soon after, Kenpachi Zaraki arrives to help them. Nnoitra and Kenpachi engage in an increasingly destructive battle, both finding great enjoyment in their ability to survive each other's attacks and prolong the fight. After a prolonged and devastating battle, Nnoitra dies what he believes to be a "warrior's death"; his life ends a split second before his body hits the ground.
