Isane Kotetsu serves as the lieutenant of the 4th Division, which is responsible for medical and supply relief within the Gotei 13, under Captain Retsu Unohana. She is the elder sister of Kiyone Kotetsu, who holds one of the two 3rd seats in the 13th Division. In contrast to her more exuberant sibling, Isane tends to be quiet and observant, though she shares Kiyone's deep sense of loyalty. Isane looks up to Unohana both as a mentor and a maternal figure. It has been noted by Unohana that Isane frequently experiences strange and recurring nightmares. These dreams often have whimsical or bizarre themes, such as one involving fish paste, and are occasionally used as light-hearted jokes. Interestingly, Isane's given name includes the kanji for "brave," while her sister's name contains the kanji for "quiet." Isane's zanpakutō is named Itegumo, which translates to "Frozen Cloud" or "Frozen Snow." The release command for Itegumo is "sprint" (奔れ, hashire), meaning "run" in English. The guard of her sword is shaped like a snowflake, and when it enters its shikai state, two smaller blades emerge from the hilt at 45-degree angles. Despite this, Isane has not been seen using Itegumo in combat; the only time she activated it, she was promptly knocked unconscious by Ichigo.
