Hiluluk's primary dream was to heal the people of the island from their frozen hearts, a condition brought about by Wapol's selfishness. After Wapol decreed that any doctor not under his control would be considered an outlaw, Hiluluk and Doctor Kureha were the only free doctors left. However, unlike Kureha, Hiluluk often caused more harm than good to his patients unintentionally.
One day, during a blizzard, he encountered Chopper. Initially, Chopper tried to fend him off, but Hiluluk gained his trust by showing that he carried no weapons and had no intention of harming him. From then on, they developed a father-son relationship, traveling across the island in an attempt to cure patients, which often led them to flee for their lives from angry patients or their families.
In his early years, Hiluluk was a thief who was diagnosed with a fatal illness. He believed that the sight of a grove of cherry trees cured him. This experience transformed him, and he decided to return to Drum Island, his birthplace, to help cure a severe illness affecting the island.
A year later, knowing he was terminally ill, Hiluluk reluctantly sent Chopper away to spare him from witnessing his death. When Chopper discovered the truth, he embarked on a quest to find an Amiudake mushroom. Upon his return, battered and bruised, this act deeply moved Hiluluk. Unbeknownst to Chopper, the mushroom was poisonous; however, Hiluluk consumed it anyway, touched by Chopper's gesture. (Hiluluk always referred to the pirate emblem as a symbol of vitality, which Chopper misunderstood, not realizing that the crossbones symbol in his medical book indicated the mushroom was toxic.)
Later, Hiluluk heard that Wapol's personal 20 doctors, the Isshi-20, were sick and rushed to the castle, only to discover it was a trap set by Wapol to lure him out and kill him. As Chopper approached the castle, Hiluluk thanked him for the wonderful life they shared. Realizing his time was near and that his country needed healing, Hiluluk drank one of his failed medicines instead of the poisonous one Chopper had prepared, resulting in an explosive end.
