Kaoru Hitachiin is a playful and mischievous first-year student who delights in teasing his fellow Host Club members, particularly Haruhi and Tamaki, whom he often calls "My Lord." He and his older twin brother, Hikaru, are known as "little devil" type hosts, performing an act of "forbidden brotherly love" to attract clients. In this act, Kaoru plays the more submissive role. Unlike Tamaki, the twins are strategic in their interactions with customers, driven by their intense bond and reluctance to let others into their world.

Hikaru and Kaoru share a profound emotional connection, formed during a lonely childhood where no one could tell them apart except for the parting of their hair (Hikaru's parts to the right; Kaoru's parts to the left). Haruhi becomes the first person able to distinguish between them without looking at their hair, which makes her friendship invaluable to them. Haruhi notes that despite being identical twins, Hikaru and Kaoru have distinct personalities. Kaoru is generally kinder, more mature, and more considerate of others.

Tamaki convinced the brothers to join the Host Club during their final year of middle school by persistently guessing which one was Hikaru. Eventually, they gave in and tried out the club, marking their first step towards broadening their social horizons. Later in the manga, both Hikaru and Kaoru develop feelings for Haruhi. In chapter 52, Kaoru takes Haruhi on a date to an amusement park, confesses his love, and kisses her cheek. However, he ultimately steps back from pursuing her because his relationship with Hikaru is more important to him.
