I'm pretty sure it's a non-issue. Like, at least from everyone I've ever known, people know the difference between presentation and expression vs. sexual orientation. Like, I've never known anyone that thought that liking pink or women's clothes makes you gay or anything like that. I think that would've been an issue like 40-50 years ago, where people would basically tell people they were if they did anything outside the norm. Same with gender identity. Like, gay and trans people, at least all of the people I've known, knew what their orientation and identity were, and didn't get them confused with other things. Like, most people know the difference between lesbians, tomboys, and trans men, and that they aren't the same thing at all. Like, I'm a tomboy, and I do like girls, but I'm not transmasc and I definitely don't wanna be a dude. I suppose a lot of older people don't know the difference though
