These type of story never really go the way of hard magic systems, so you can think up whatever explanation you like. If I was creating my own magic system I would put a restriction in a way that human body has some sort of innate resist, which restrict forming spells or something like that.
I've read stories where wizard could create molten lead inside in other person throat, however I never really liked magic systems which doesn't have certain restrictions. I prefer magic system a bit more similar to martial arts like Avatar for example.
