Mutsumi Otohime is a gentle, frail, clumsy, and soft-spoken 21-year-old woman residing in Okinawa. She frequently experiences fainting spells that make her seem as though she has passed away. Others describe her as sisterly, kind, caring, yet somewhat scatterbrained. Like Keitaro, she is a third-year ronin aiming for Tokyo University. However, her failures are often due to simple mistakes such as forgetting to put her name on the test or fainting during the exam because of her anemia.
In both the manga and anime, Mutsumi, along with Naru, was a childhood friend of Keitaro and had promised to attend Tokyo University together. She has a fondness for watermelons and turtles, even gifting a turtle named Tama-chan to Keitaro and Naru. Mutsumi comes from a large family with eight siblings and bears a strong resemblance to her mother. She is often referred to as the "female version of Keitaro" due to their shared clumsiness.
When Kanako takes over the management of the Hinata house, Mutsumi disguises herself as Keitaro to test the girls' hostess skills. Her resemblance to him is so striking that even Naru is momentarily fooled.
