Black Jack is a medical mercenary, offering his expertise to the highest bidder. He operates in the shadows, clad in a black cloak, with striking black-and-white hair, a scarred face, and partially darkened skin. Black Jack treats patients from all walks of life indiscriminately, ranging from everyday people to presidents and yakuza leaders. For his VIP clients, he demands exorbitant fees. This has earned him a reputation for being callous and greedy, an image he enjoys fostering. Yet, to the reader, it becomes evident that Black Jack is fundamentally a good person: he opposes wealth and prestige, believing he does a service to the rich by stripping them of their material possessions. This opposition to wealth and power is a recurring theme in Tezuka's work, where powerful men are often depicted in a negative light.
