Balalaika is the leader of Hotel Moscow. Her real name is Sofiya Pavlovna. She was raised by her grandfather, who was a high-ranking military official in the USSR. Before becoming involved with the mafia, Balalaika served as a captain in the Soviet Army and was a member of the Vozdushno-Desantnye Vojska paratroopers, participating in the Soviet war in Afghanistan. Known for her sharpshooting skills, she earned the nickname "balalaika," a slang term used by the Soviet Army for the Dragunov sniper rifle. Thanks to her leadership during fierce battles in Afghanistan, her soldiers hold her in high esteem and are willing to follow her orders without question. As a combat officer, she is a skilled strategist and versatile fighter, capable of handling herself despite the frequent concern from her troops, including Boris. Though she can be cynical at times, she is also a pragmatic and professional businesswoman. She often hires Dutch and the Lagoon Company, likely due to mutual respect.
Balalaika's appearance is marked by burn scars covering much of the right side of her face, neck, and chest, injuries sustained in Afghanistan, which led to her derogatory nickname "Fry-Face." She has long blonde hair and typically dresses in a military greatcoat over a low-cut business suit. Unlike other characters who smoke cigarettes, she prefers cigars. Based on her service in Afghanistan and missing the 1984 Summer Olympics in Los Angeles, she is estimated to be between 31 and 35 years old. She is one of the few individuals who have earned Revy's respect. In turn, Balalaika seems to value Rock, sometimes relying on his knowledge and business acumen.
For critical operations, Balalaika calls upon a personal group of ex-Soviet airborne troops and Spetsnaz commandos who are veterans of the Afghan conflict. Known as the Desantniki, they serve as Hotel Moscow's special ops team. When conducting operations from Thailand, she uses the Russian cargo ship Maria Zeleska as legal cover from police and law enforcement agencies. Despite being on the Interpol list, she operates under the pseudonym Vladilena N. Vasilinov in Japan, protected by diplomatic immunity.
