Sakurako was relentlessly tormented by her schoolmates due to her perceived ugliness from birth. Her confession of love to Domyouji only resulted in further trauma as he mocked her appearance. She had a friendship with Makiko during elementary school, but her family's business necessitated a move to Germany. After undergoing plastic surgery, Sakurako returned to Eitoku High School, resolved to leverage her newfound beauty to exact revenge on Domyouji by making him fall for her.
She encountered Tsukushi when seeking help at Shizuka's birthday party. Sakurako maintained a facade of sweetness and innocence, but revealed her true nature when she believed no one was watching. Upon learning about Domyouji's growing interest in Tsukushi, Sakurako conspired with her friend Thomas to ruin Tsukushi’s reputation. They drugged Tsukushi at a club and circulated photos of her in bed with an unknown man, causing the student body to turn against Tsukushi for allegedly betraying Domyouji.
When Tsukushi discovered Sakurako's involvement, Sakurako admitted to using her solely to get back at Tsukasa, asserting that people like Tsukushi—plain and average—shouldn’t associate with handsome men like those in the F4. Once Domyouji rejected Sakurako's advances and saw through her schemes, she felt remorseful. However, the entire student body then began bullying her over her plastic surgery, but Tsukushi stood up for her. Despite this, Tsukushi declared she couldn't forgive Sakurako.
Sakurako attempted to use Tsukasa's breakup with Tsukushi (due to Tsukushi's romance with Rui) to get closer to him. However, Tsukasa pretended to be close with Sakurako just to make Tsukushi jealous. Initially, it was unclear whether Sakurako and Tsukushi could ever reconcile their differences, especially since Sakurako became cruel around Tsukasa. As the story progressed, however, Sakurako appeared more amicable, even defending Tsukushi against others with malicious intent.
