Angelina, known as the widow of the late Baron Barnett and a queen within high society where she is nicknamed "Madam Red" due to her hair and attire color, is also Ciel's aunt. Beyond her unannounced visits to Ciel's London residence or her flirtatious interactions with admirers, Angelina works as a rare female doctor, mainly serving female and child patients. Although she has no children of her own, she once told Ciel that she considers him like her own son and has been watching over him since his return after the fire incident. She employs a butler named Grell, whom she frequently scolds for not being more like Sebastian.

Both Angelina and her older sister Rachel were in love with Ciel's father, the previous Earl Phantomhive. The Earl was drawn to the bookish and shy Angelina, praising her red hair which she initially disliked. This compliment made Angelina fall in love with him, leading her to adopt a more stylish appearance and less reserved demeanor in hopes of an engagement. However, the Earl chose to marry Rachel. Despite this, Angelina did not harbor resentment towards her sister's happiness and enjoyed spending time with the couple and their son, Ciel. Eventually, she became engaged to Baron Barnett who loved her despite knowing about her lingering feelings for Earl Phantomhive. Over time, she became pregnant.
Tragedy struck when a runaway carriage killed both Angelina and her husband. While he died instantly, Angelina survived but lost her unborn child and suffered internal damage that prevented future pregnancies. Rachel cared for her in the hospital and invited her to Ciel's tenth birthday upon her release. Angelina arrived at Phantomhive Manor just as it was burning down.
Shortly after, one of her patients sought an abortion, declaring children worthless. This triggered Angelina, leading her to murder the woman that night. Grell, seeing potential in her, posed as her butler and together they committed murders as Jack the Ripper. Ciel eventually uncovered the truth and confronted them with Sebastian. She nearly killed Ciel but stopped upon seeing her sister's face in him. Grell urged her to continue, but she refused, stating she couldn't kill Ciel. Consequently, Grell killed her, deeming her no longer fit to wear red.
Madam Red is loud, outspoken, and flamboyant. She delights in teasing people, from praising Ciel in dresses to shamelessly groping Sebastian. At social events, she revels in the attention from her admirers. Despite her upper-class status, she can be surprisingly crude, leading to censorship in some stories. She often criticizes Grell for underperformance. Nonetheless, she genuinely cares for Ciel, trying to mother him in her unique ways. As a doctor, she leverages her education in various schemes.
Her grief and rage over losing her husband, child, sister, and the man she loved have left her slightly unhinged, though hidden well. She vents this anger on women who, she believes, don't value their ability to bear children. Despite her actions, she appears shaken, driven by Grell's encouragement.
At her funeral, Ciel refers to her as "Aunt Ann".
