I suppose there could be multiple issues here. Let's start assuming it's the author of the manga:
>Has been commissioned to adapt it the fastest way possible for whatever dumb reason you can think about it
>Has been commissioned to just adapt it but the publication size is too small so he has to cram everything on so little space
>They hired him and has no literal idea what's he doing
Now what about the LN/webnovel or whatever author?
>Wants the publication to catch as fast they can to sell his original source material
>Has been messing around one way or other with the manga ("Fixing" a little here and there if you will)
>The story... it's just trash. No matter who's working on it this is just some godawful story and it's hard to make a readable thing out of it.
