All things considered, Kaoru's fake death was handled decently. It's arguable whether or not her dying would have been better for the story or not, but TO ME Kenshin is to likable for me to want to see him suffer anymore. We get it, he did some horrible things in the past I think killing off Kaoru would have been overkill.
Enishi himself is a very deranged person but I do consider him a much more effective villain than Shishio Makoto was...to just came off as bad for the sake of it.
