Judging by that whirlwind, I'm guessing that Gut's new despair event horizon is triggering the Behelit he's been carrying all this time? I guess it's about time for that thing to finally play a role after so many real life years of just sitting in Gut's possession. Though none of the Behelit triggering moments in flashbacks ever showed Trolls or Qliphoth to manifest (it always implied it merely sent the user and their potential sacrifices to the limbo dimension the God Hand reside in and that was it) so I do worry if that's just an extra bit of lore ignoring on the writing part for the sake of some visual action.
